Each room features a clean and modern look with a kitchenette, table, television and plenty of room for friends and family to visit, and for you to rehabilitate.

Shady Lane’s Rehab is the support you need with the amenities you want. We strive for excellence here at Shady Lane and have a 5 Star rating from the Centers for Medicare & Medicaid Services (CMS) for annual health inspections, staffing ratios, and quality measures. 

Dr. Kirk Dimitris, M.D

Dr. Dimitris, a board-certified orthopaedic surgeon for Lakeshore Orthopedic, is Shady Lane’s Orthopedic Program Consultant.  He specializes in the comprehensive treatment of hip and knee arthritis, including both conservative and surgical care.

Dr. Dimitris comes to Shady Lane’s Rehab four times a year to conduct presentations where you can have your questions answered about knee or hip replacement.

 

HFM Shady Lane Clinic

The HFM Shady Lane Clinic, offered through Holy Family Memorial, is conveniently located and accessible to the general public and residents. Dr. Cheryl Pitre provides a full range of primary and preventive services. Clinic hours are Monday, Tuesday, Wednesday, & Friday 7:30 a.m. – 4:30 p.m. Call 920-320-2436 to schedule your appointment. 

Devoted & Experienced Therapists

Because the client-therapist relationship is important to a continuity of care, the added advantage for our Rehab clients is the ability to continue working with their therapist after they leave Shady Lane’s Rehab.

 

Short Term Stay

If you’ve had a setback in your health, you don’t feel comfortable going home alone after a medical procedure, when a care-giver will be away, or when respite care is needed, that’s when you come for a Short Term Stay. 

Stay for a few days or up to thirty days until you’re ready to return home. You’ll have the option of customizing your own services making this a great value.
